2 Positions available: Clinic/Hospital POSITION SUMMARY The Certified Diabetes Educator (CDE) is a skilled and experienced professional Registered Nurse (RN) responsible for excellence in the provision of Diabetes Education for patients in the inpatient clinical areas and ambulatory care service line. Reporting to the Director of Professional Practice and Education and accountability to the Director of Ambulatory Services, the Clinical Educator is accountable for the education program planning for staff and patients in the hospital and outpatient clinics. The Certified Diabetes Educator incorporates considerations of Age and Population Specific awareness and the Watson Model of Care in all program planning. The CDE is responsible for developing and implementing the inpatient and outpatient diabetes program for MLKCH. The position requires the full understanding and active participation in fulfilling the Mission of Martin Luther King, Jr. Community Hospital. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Martin Luther King, Jr. Community Hospital's strategic plan and the goals and direction of the quality and performance improvement process activities. ESSENTIAL DUTIES AND RESPONSIBILITIES 1.Establishes an education program for diabetes prevention and the proper management of patients with diabetes in the inpatient and ambulatory care setting. 2.Keeps the Director of Professional Practice and Education and Director of Ambulatory Services informed of educational activities, status of programs, and proposals for new and revised programs. 3.Participates as a member of interdisciplinary healthcare team to facilitate the continuum of care and education of patients with diabetes. 4.Serves as a consultant/resource to the clinical areas, clinic staff, home health, and departments as it relates to the diabetes diagnosis. 5.Works with the nursing leadership team for new clinical initiatives regarding diabetes. 6.Cultivates positive and collaborative relationships within the Education Department, nursing, and clinical leadership team to ensure that educational needs are being met in the most effective manner possible. 7.Provides leadership through educational activities to bring about changes in clinical practice regarding diabetes. 8.Evaluates education programs on a regular basis and plans and implements improvements based on this evaluation and feedback from customers. 9.Monitors and evaluates, as needed, resource utilization, patient outcomes, patient’s skills in self-care, and patient’s ability and readiness to learn. 10.Ensures correct and timely documentation inpatient’s medical record following individual consultation. 11.Counsels and educates individual patients appropriately on diabetes care in conjunction with patient’s medical provider (medication adjustment, exercise, diet, sick days, etc.). 12.Evaluates blood glucose records to determine efficary of current medication therapy, solve hypo/hyper-glycemia problems, and make needed adjustments. 13.Utilizes adult learning principles in program planning to select the teaching strategy and method appropriate for the learner and the content including e-learning. 14.Develops and conducts group presentations and related educational programs, and staff inservices, as needed. 15.Plans for all aspects of programs offerings: curriculum development, objectives, handouts, speakers, continuing education credit, room(s), food and assistance with presentations, as needed. 16.Develops and maintains patient education materials relating to diabetes. 17.Plans for the marketing of all programs to appropriate units and/or departments. 18.Ensures the curriculum utilizes the information generated from the Quality and Performance Improvement activities and MLKCH nursing and clinical Policies and Procedures as a foundation and incorporates these components into the diabetes education provided to the staff. 19.Uses, protects, and discloses MLKCH patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards. 20.Performs additional duties as assigned.

POSITION REQUIREMENTS A. Education Bachelor of Science degree in nursing required; Master’s in Nursing or Nursing related preferred. B. Qualifications/Experience Minimum of two (2) years of clinical nursing experience and a minimum two (2) years diabetic experience with inpatients and outpatients. Current California Nursing license. Certification in Diabetes Education. Experience with implementation of an EMR preferred.
