In this role, you will collaborate with physicians to develop age appropriate educational plans, perform assessments that include the ordering and interpreting of diagnostic services, physical exams, and provide therapeutic treatments according to age specific therapies.
Performing comprehensive assessments on patients
Analyzing data and concluding diagnosis with plans of care
Effectively coordinating and communicating plans of care
Documenting assessments, plans of care, education and discharge planning
Identifying patient/family needs to develop appropriate teaching plans, including counseling and referral to hospital based and/or community-based resources
Ordering age appropriate screening tests, lab results and diagnostic procedures with the intent to define diagnosis and prognosis
Contributing to the development of nursing standards and performance improvement activities
Teaching in the use of blood glucose meters, insulin delivery devices and continuous glucose monitoring systems
Interpreting blood glucose results and discussing adjustments in food, medication or physical activity
Participating in data collection and analysis to meet evaluation system objectives
Handling other related duties as assigned
Bachelor’s Degree in Nursing (BSN) or Nutrition Science
2-3 years of related work experience
Current NJ RN License or Certification as a Registered Dietitian
Current Diabetes Educator Certification (CDE) by NCBDE
Superior analytical, problem-solving, customer service, organizational and oral/written communication skills
