The Patient Navigator, reporting to Principal Investigator and Project Manager, will monitor Hepatitis B (HBV) and Hepatitis C (HCV) screening in the ED and will work with project implementation team to expand the HBV HCV screening program within the ED and other hospital service lines. Through daily Cerner reports the Patient Navigator will identify positive patients, contact those patients with positive test results, and educate them on viral hepatitis, importance of follow up care, and provide linkage to care with a HBV or HCV provider and with primary care if absent. Linkage to care will also include providing follow-up to ensure engagement in care and provide patient support. The Patient Navigator will utilize a referral network of primary care, specialty care, social services while developing relationships with federally qualified health centers and other community-based organizations.
The Patient Navigator will provide education and support to staff in identifying eligible patients, the importance of Hepatitis testing, and general support to the logistics of the screening program. The Patient Navigator will work with physicians and nurses to provide support in answering patient questions and provide counseling as needed.
Patient Navigator will work with the Project Manager and IT as needed to ensure data collection and integrity with respect to patient volume, patient demographics, testing rates, positive test results, and patient declinations. The Patient Navigator will maintain database of positive patients and compliance with first linkage to care appointment.
EDUCATION: Minimum Bachelor's degree in a relevant field required.
REQUIREMENTS: A minimum of two years of professional experience in a health program, which included responsibilities for patient education and linkage to care; prior experience in infectious diseases is helpful. Demonstrated computer skills with Microsoft Office Suite.
