Lead Financial Counselor (Social Worker preferred)
JOB SUMMARY: Develops financial plans for patients who may be uninsured or under-insured with minimal guidance. Screens patients and facilitates referral to state/other funding resources. Conducts follow-up on in-house and facilitates referral to state/other funding resources. Conducts follow-up on in-house and discharged accounts.Assists with coordination of post discharge care appointments as part of the discharge process for patients who are hospitalized at VUMC. Collaborates with the health care team to coordinate post discharge care appointments for patients, with a focus on removing barriers and promoting efficient and effective use of resources under the direction of the (new department name, say 'Department of Patient Referral Assistance' and the 'Office of Hospital Referral Assistance (OHRA)') social worker and attending physician. Assists patients in establishing patient specific (Discharge appointment) goals, both long term and short term. Facilitates appropriate follow up by referring patients to the appropriate next level of care either within VUMC or within the community. Serves in an advocacy role on behalf of patients. Assists in meeting key Pillar outcomes and system improvement goals including financial and satisfaction.Provides guidance and assistance to patients by orienting patients on the OHRA's processes and procedures. Follows patient while inpatient and post discharge for ongoing appointment needs until all requested care directed by the attending physician has been set up to ensure that the patient has successfully engaged in care.This position Improves outcomes by assisting to reduce hospital readmissions. Participates in warm handovers to the next level of care by providing timely, pertinent information in a standardized way; conduct patient education on discharge appointments. Assists with department wide change initiatives, quality (monitoring, initiatives and coaching).
Provides intake assessment for the Office of Outpatient Referral Assistance (OORA) calls and referrals. Assists with coaching for staff and physicians on the role of OORA as part of the VMG and Patient Financial Services. Assist with documentation and collection of statistics on outcomes with patients.
Conducts interview of uninsured and out of network patients
Gathers information regarding the patient's primary care physician (provider name, clinic name and phone number), assess for information about other providers, medical clinics and facilities the patient has seen regarding their current condition, and outlines patient transportation resources.
If patient does not have a PCP, assists with finding a PCP and setting a PCP appointment within the requested timeline per referring physician. Assists with developing community resource options under the direction of the OORA manager and referring physician.

Ability to understand, read and interpret a medical record, including both clinical and non-clinical information. Ability to share information both verbally and in writing with physicians, staff, as well as, to utilize same to advocate with other medical facilities and insurance companies.
Ability to do assessments of a patients, including psychological, biological, social and financial.
Ability to develop financial plans for patients who may be uninsured, underinsured or out of network with minimal guidance. Ability to understand and detect insurance nuances as related to patient's medical care needs, and in turn be able to communicate insurance information to both patients and physicians in a way they are able to understand - both verbally and written.
Ability to assist physicians or nurse practitioners upon request in facilitating appropriate access and care for uninsured underinsured and out of network patients by providing information and assistance on when to schedule the patient with a VMG physician or nurse practitioner vs. when to refer the patient to a non-Vanderbilt resource.
